1) Make your appointment by clicking

2) Please print and fill out these forms.

3) Bring the forms along with you to your 1st visit.

(Bring along your BC Care Card/Drivers License and Insurance/Benefits info or card too, this will help out, so we can direct bill your insurance).

4) Arrive 10 min early because you have already filled the forms out. Hand in the forms and benefits cards to our wonderful receptionist. She will take the info and see what your coverage is.
